LONGTERM RESULTS AFTER INTRAVITREAL INJECTION OF ISIS 2922 IN PATIENTS WITH CMV-RETINITIS AND AIDS.

In all manifestations of CMV-retinitis in patients with AIDS is the most common and severe complication which can result in blindness untreatedly. Our study shows effects and complications after a longterm treatment with the new drug ISIS 2922 (FomivirsenTM).
Patients and Methods: We have been treating 5 patients in a randomised controlled study with ISIS 2922. Evaluation period ranged from 0,5-2,5 years in 1996-1999. We injected 0,05 ml Fomivirsen with the dosage of 6,6 mg/ml intravitrealy. Group A was treated every 2nd, group B every 4th week. All patients were treated with topical antiseptic eye-drops after each injection to reduce a risk of infection. We checked visual acuity, effectivity and complications every visit.
Results: 6 eyes of 4 patients were injected in a 4 week rythm, 2 eyes of one patient were treated every 2nd week. Before the beginning of the study visual acuity ranged from 0,5-1,2 (mean 0,9). Results at the end of the trial ranged from counting fingers to 0,7 (mean 0,28). The decreasing of the visual acuity was a result of retina detachment in 2 eyes, cataract in 3 eyes, atrophic optic nerve in 1 eye and intraretinal bleading in 1 eye. Intraocular pressure was in normal range (mean 14 mmHg). We didn`t see any progressing CMV-retinitis in any eye of all the group.
Conclusion: The new oligonucleotide Fomivirsen shows good results in treatment of CMV-retinitis. Complication can be handled with cataract or vitreoretinal surgery in most cases. Antiseptic eye drops should be administered after every injection to reduce the risk of infection.
