LASER-FLARE IN COMBINED CATARACT UND GLAUKOMA SURGERY

In combined cataract and glaucoma surgery insufficiency of filtering bleb is higher than in pure filtering surgery. As a reason for this a higher exsudation of fibrin in combined surgery is considered. We examined the inflammation of the anterior chamber in combined cataract and glaucoma surgery, pure filtering surgery and cataract extraction with a laser flare meter.

Patients and methods: In consecutive series we carried out a combined glaucoma surgery and a cataract extraction with phako emulsifikation and implantation of a foldable intraocular lens via the same incision in 34 patients. We examined vision, tension and the status of the glaucoma and performed an examination with laser flare meter before surgery, in the first and second day, three weeks and six month after operation respectively. We compared the results with 32 patients receiving a single goniotrephination or a cataract extraction via clear cornea with implantation of a foldable lens.

Results: There were no statistic significant differences between the basic dates. In patients with combined surgery the mean value of laser-flare at the considered days was 11,5; 49,2; 22,3; 15,1 and 9,2, in patients with goniothrephination 8,2; 19,4; 20,2; 7,6 and 7,2 and in clear cornea cataract extraction 10,3; 15,0; 16,5; 11,8 and 7,9. The increase of the flare in the first two days was significant in combined cataract and glaucoma surgery and in single goniotrephination. In the patients with single cataract extraction the increase of flare was not significant. Three weeks later, there was no significant change in flare in comparison to the values before the operation.

Conclusion: We found the highest flare on the first day in combined cataract and glaucoma surgery as a sign for protein exsudation in comparison to a single goniotrephination (p<0,05). This might be the reason for the more frequent scarring of the filtering bleb in combined surgery.
