Phakoemulsifikation of the human lens by the waterjet

Backround: Ultrasound phako of cloudy lens today is gold standard in cataract surgery. Nevertheless new technologies are used to reduce the endothelial cell loss, the heat of the tissue, the diameter of the openings. There were discussed bimanual phako, laser phako, Katarhex - and the water jet too.

Methods: Human during ECCE obtained lens nuclei in water bath were divided using the waterjet. Different parameters like pressure, distance and angle to the nucleus were modified.

Results: The consistence of the nuclei varied. All nuclei are divided by directly water jet during 10 seconds up to pressure of 10 bar.

Discussion: To divide especially hard human lens nuclei waterjet pressure were necessary higher then the pressure tolerated by the lens capsula. The use of a waterjet with this high pressure in introcular surgical procedures are not possible. Wter jet phako instruments without danger for the lens capsula are neccessary.
