Lastest results of macular rotation

Between February 1997 and January 2000 we performed macular rotations on 82 patients. Here we report on the complications and results.
Materials and Methods: Artificial retinal detachment, 360 degree retinotomy and removal of the choroidal neovascularization membrane (CNV) was followed by a rotation of the retina upward around the optic nerve (35 degree on average). In almost all patients, muscle surgery was additionally performed to compensate for the incyclotropia caused by the retinal rotation.
Results: Postoperatively the silicone oil could be successfully removed from all eyes, in the majority of eyes already 10 days after macular rotation. In 24 eyes (29%) a PVR-reaction occurred, usually combined with a retinal detachment which made one or more additional operations necesssary. A cystoid macular edema occurred in more than half of the cases which were operated on in the first two years. In eyes which were operated on later the cystoid macular edema occurred less often. Fourteen eyes (17%) developed a recurrence of the CNV. At final examination 25 eyes had a far visual acuity of 0.4 or better and 39 eyes had a near visual acuity of 0.4 or better (Oculus charts).
Conclusions: In the majority of the cases macular rotation succeeded in restoring foveal function. A reading vision was achieved in 47%. With respect to the longer follow-up period the results continue to be encouraging.
