Mandic, R. IvekovicIntroduction: In a prospective study the controversy of conjunctival flap orientation in combined phacotrabeculectomy has been evaluated. Advantages and disadvantages of limbus-based versus fornix-based conjuntival flaps has been analysed.
Methods: Phacotrabeculectomy with the conjunctival flap orientation has been performed in 18 patients with cataract and glaucoma on one eye. In the same patients on the second eye the equal operative technique has been performed but with fornix-based flap. Preoperative and postoperative visual acuity and intraocular pressure, as well as postoperative complications were studied.
Results: After 14 months, at last follow-up visit, visual acuity 0.5 or better in fornix-based group was noticed in 86,5% eyes and in the limbus-based group in 75,7% operated eyes. Preoperative mean intraocular pressure in limbus-based eyes was 20.6+4.5 mm Hg on a mean 2.2+1.1 antiglaucomatous medications. In fornix-based group preoperative intraocular pressure was 21.3+4.4 mm Hg on a mean 2.3+1.4 medications. In postoperative follow-up in limbus-based group intraocular pressure decreased to 15.4+4.4 mm Hg (P<0.01). Mean postoperative intraocular pressure in fornix-based group was 16.2+3 mm Hg (P<0.01).
Conclusion: Phacotrabeculectomy with limbus-based or fornix-based flap are equally effective according to in improvement of visual acuity, IOT, decreasing application of antiglaucomatous medicaments as well as in number of postopertive complications.
