Inflammatory component in hereditary cornea stromal dystrophies

One of the couse of the reccurent erosiones (RE) in patients with hereditary stromal cornea dystrophies (HSCD), exept degenerative changes, is inflammatory process, the appearance of wich degeneration.
The porpose of investigation was to study role of imflammatory component in development of progressive secondary degenerative changes in cornea of patients with HSCD.
Patients: Clinical and morphological investigations were performed in 122 patients (244eyes) with HSCD, among them 42 - with granulare(Groenouw 1), 63 with lattice (Type 1), 17 with macular (Groenouw 2) corneal dystrophy aged 15 65 years ( M=41,6± SD12,5) ears. Morphological investigation were conducted in corneal disk after lamellare keratoplasty..
Results: results showed that inflammatory processes such as recurrent erosions and keratitis accelerate progression of secondary degenerative changes in cornea, with cause early decrease of visual acuity. Direct correlation between frequency of recurrence inflammatory processes (RIP) and kind of dystrophy was established (x2=32, p=0,00007). Recurrence erosions and keratitis were observed in 98.6% patients with lattice, 59,2% - with granular and 70,5% - with macular stromal dystrophies.
Conclusion: In cases with no effect from conservative treatment phototherapeutic keratectomie or lamellare keratoplasty are necessary., with to remove degenerative changing epithelium, Bowmane membrane and superficial layers of stroma, which are sources of recurrent inflammatory processes.
