ICD-10aug – a differentiated ophthalmological classification of diseases

Introduction: Coding of diagnosis of all outpatient and inpatient cases using the ICD-10 SGB V was introduced in the German health system in January 2000. A more detailed coding for clinical and scientific purposes can be desirable especially in hospitals. The use of a separate more detailed coding system for these purposes is senseless.

Method: Up to two more digits were added to the ICD-10 SGB V, a German adaptation of the ICD-10, in order to differentiate all diagnosis relevant to the field of ophthalmology. The recommendations for specialised editions of the ICD-10 published by DIMDI were fulfilled. The actual release (ICD-10aug, 5.0) includes the ICD-10aug as it has been used in our department since 1993 and published on the WWW in 1998 (www.dog.org/icd10), own experiences with coding, recommendations of colleagues and search strings sent to the webserver of the DOG. A systematic list and a alphabetical thesaurus have been developed and are available in print version and on the WWW.

Results: The first 4 digits of the ICD-10aug are fully compatible with the ICD-10 SGB V. Thus coding for administrative purposes can be generated by deleting the 5th and 6th digits. The ICD-10aug includes 3000 codes, 1800 codes in the 5-digit and 700 in the 6-digit extension

Discussion: The ICD-10aug is a detailed classification of diseases that is suitable for administrative as well as clinical and scientific purposes. It is public domain. As it is fully compatible with the ICD-10, disadvantages like the clinically unfavourable basic structure and lag of multiaxial coding can not be eliminated.
