Pars plana vitrectomy and scleral-sutured posterior chamber lenses (pcIOL) for different indications

Introducation: Suturing of pcIOLs in complcated aphakic settings has become a routine procedure. During a pars plana vitrectomy (ppv) in the absence of a retinal detachment, this surgery has only been indicated with caution.

Patients: We demonstrate four eyes undergoing a ppv becuase of a luxated lens, luxated pseudophacos, intraocular foreign body and endophthalmitis. The surgical procedure comprised primarily a clarification of the intraocular condition by ppv, in some cases combined with a prior lensectomy or extractoin of an intraocular lens, followed by an implantation of a Morcher 81E pcIOL via 2 prolene sutures under lamellar scleral flaps by a sceral or a corneoscleral tunnel. The fluid-air exchange was done at the end of the surgery, and 0.4ml C2F6 were instilled.

Results: The aphakic conditions without a pcIOL allowed for a clear visualization of retinal lesions, in order to perform a complete and adequate ppv and endolaser coagulation. The pcIOL implantation at the end of surgery bears the advantage not to disturb fundus visulization by optic refractory phenomena. The large optic and flexible haptics are advantageous concerning the postoperative risk of an iris capture.

Discussion: The implantation for a sulcus fixated pcIOL during a ppv may be advantageous with respect to a fast postoperative visual rehabilitation.
