Long term follow-up of HLA-A and -B matched high-risk corneal transplantations

Objective: Evaluation of long term results of high-risk transplantations and investigating the influence of HLA-A and -B matching on graft survival.
Methods: Retrospectively we analysed the follow-up results of 312 highrisk patients, who underwent a HLA-A and -B matched graft between 1982 and 1995 in The Rotterdam Eye Hospital. Transplant functionality and allograft rejections were documented throughout the follow-up. Kaplan-Meier method was used to determine the rejection-free transplant survival for patients with 3 and 4 HLA-A and-B matches and compared with those patients who received a donor cornea with only 2 matches.
Results: Total 5-year survival in these high risk cases was 59,3 %. When the data were analysed on split-matches, the group with the better matching performed significantly better, especially during the first 3 years after transplantation.
