Retinal arterial macroaneurysms - a retrospective analysis

Objective: Aim of the study was a retrospective analysis of the clinical features, therapy and clinical course of patients with retinal arterial macroaneurysms.
Methods: Patients with retinal arterial macroaneurysms were identified by screening our department's fluorescein angiogram records of the last 8 years. Patient records were evaluated with respect to ophthalmologic and general risc factors, clinical findings, course and therapy.
Results: Overall 19 patients with a retinal arterial macroaneurysm were identified, among them were 14 female and 5 male. The age range was 47 to 87 years (median 73 years). 2 female patients developed two macroaneurysms each with several months time interval. Therefore, 21 macroaneurysms were analyzed. Localization of the aneuysms was as follows: superior temporal artery 8/21, inferior temporal artery 11/21, superior nasal artery 1/21, and 1/21 directly at the optic disc. In 10/21 aneurysms the clinical picture was predominated by exsudation and in 10/21 by bleeding. One macroaneurysm showed neither exsudation nor bleeding. 15/21 eyes were treated with perianeurysmatic Argon laser coagulation. Median follow up time was 27 months (range 2 to 52). Visual acuity significantly improved in 9 eyes, was unchanged in 9 eyes, and deteriorated in 3 eyes. Of the eyes treated with Argon laser coagulation visual acuity improved in 6 eyes, was unchanged in 6 eyes and deteriorated in 3 eyes. Of the untreated eyes 3 improved and 3 were stable in their visual acuity.
Conclusions: With respect to age and sex distribution our results confirm those of other authors. A definite recommendation for the indication and value of argon laser coagulation can not be given on the basis of these data.
