Bioptik: A procedure of refractive surgery for treatment of high myopia

Background: Correction of high myopes to emmetropia can be done only by very few methods. The combination of the ICL implantation with LASIK (Bioptic) is considered to be appropriate to achieve emmetropia even in high myopic eyes.
Patients and Methods: 18 eyes of 9 patients have been treated by the Bioptic procedure. After performing corneal dissection with the Hansatome an implantable contact lens (STAAR) was introduced into the posterior chamber. Four weeks later the residual ametropia was corrected by the use of an ablative keratectomy.
Results: Preoperative manifest refraction measured -16.63 ± 4.65 D sph combined with -1.38 ± 1.28 D cyl and was reduced to -4.35 ± 2.20 D sph combined with 1.65 ± 0.83 D cyl after implantation an ICL. Thereafter the uncorrected visual acuity increased from hand motion to 0.13 ± 0.10 and including best correction from 0.53 ± 0.19 to 0.63 ± 0.19. The average refraction of implanted ICL was -14.6 ± 1.10 D. Best corrected visual acuity increased to 0.79 ± 0.20 after the completion of the procedure. The ablated depth of the cornea was 80.63 ± 26.9 µm and the optical zone measured 6.28 ± 0.39 mm.
Discussion: Bioptic is a combination of two different methods to reduce myopia. It can be used as an additive aid to treat cases in whom a single method is not valid or problematic. A further advantage of Biotic is the larger optical zone which should reduce glare and night driving difficulties. With bioptic the contact of the steep edge of the ICL with the pigment epithelium should be reduced. A preexisting astigmatism can be treated during the LASIK procedure.
