Phototherapeutic keratectomy in superficial corneal dystrophies management

Smoothing of corneal surface irregularities is the main indication to phototherapeutical keratectomy perfomance. This investigation deals with efficacy of excimer laser PTK in different anterior corneal pathology.
Material and methods: Eighteen eyes (13 patients) with primary and secondary corneal dystrophies underwent PTK. The anterior corneal surface irregularity was a result of hereditary corneal dystrophies : lattice dystrophy (n=3); Reis-Bucklers (n=2) cases; reccurens of granular dystrophy in the corneal graft (n=2); Salzman's nodular degeneration (n=2), pterygium-treatment (n=3), secondary bullouse kerathopathy (n=4); combinations phototherapeutic\ photorefractive (corneal opacification + Myopie - 5,0 D, n=2). In all cases PTK with excimer laser Coherent\Schwind Keratom was performed after preliminary epithelial abrasion with hockey knife. The depth of ablatiion dependet on the depth of corneal opacification and varied from 25 to 85 mm . Diametr of ablation zone was 7,0 - 8,0 mm. The follow up period was from 3 to 25 months ( in average 11,8±8,09).
Results: Preoperative best corrected visual acuity was 0,10±0,02. After PTK, when the corneal epithelization was completed, uncorrected visual acuity increased to 0,68±0,07. The mean time of corneal reepitelization was 4,09±0,31 days. Two eyes became hyperopia + 2,0 D.
Conclusion: PTK could be a good alternative to lamellar keratoplasty in cases of superficial corneal dystrophies. Correcrion of refractive anormalities can be performed simultaneosly.
