Monocular traumatic hemianopia

Objectivs: Monocular hemianopia strictly limited to the vertical midline of the visual field is rare. Herein we describe a traumatic etiology in two patients.
Methods: Two females (seventeen and thirty nine years old) presented with complaints of unilateral nasal visual field loss after blunt injury of the ipsilateral temple and after sling injury in a car accident. A complete morphologic examination was performed as well as static and kinetic perimetry, electrophysiologic testing (M-VEP,M-ERG, MF-ERG) and imaging (cranial NMR with orbita coil and CCT).
Results: Clinical examination revealed an ipsilateral partial atrophy of the optic nerve, which was confirmed by electrophysiologic testing. Visual acuity, pupillary reflex and binocular visual field were normal. No pathology was detected by NMR and CCT.
Conclusion: A traumatic cause is rare but has to be considered among differential diagnoses of monocular hemianopia strictly confined to the vertical midline of the visual field. In the medical literature such defects are attributed to processes in proximity to the optic chiasm.
