Acute detachment of the retinal pigment epithelium during spine surgery

Introduction: We report on a patient who suffered from an acute visual loss, central scotoma and impairment of colour vision following spine surgery.
Methods: Six days after the appearance of a detachment of the retinal pigment epithelium and loss of visual acuity to 10/200 a fluorescein angiogramm could be performed.
Results: We found a detachment of the retinal pigment epithelium with irregular reflexes and late leakage as well as hard exsudates close to the inferior macular artery. Within 6 weeks all morphological and functional changes disappeared except for the hard exsudates. Visual acuity recovered to 20/25. The central scotoma as well as colour vision disturbances vanished and the light difference sensitivity normalized.
Conclusion: Besides an acute ischemia during the surgical intervention in general anaesthesia and face down position a stressreaction of the pigment epithelium with subsequent decompensation can be discussed.
