Excimer laser therapy (PTK) of recurrent corneal erosion

Background: Conventional therapy of recurrent corneal erosion with ointment or eyedrops shows rather unsatisfying results. Surgical removal of epithelium or puncture with a fine needle show a high rate of relapse, too. Phototherapeutic Keratektomy is a new method with only a few long time observations.
Patients and methods: In a restrospective study we examined the relapse rate of the recurrent corneal erosion after PTK (193 nM). Between 1994 until 2000 we treated 85 eyes, which had a recurrent corneal erosion up to 15 years. The follow-up period ranged from 4 to 86 months (mean: 40,8ms). In this time we examined 76 patients (f=39, m=37) aged between 28 and 95 years (mean: 46yrs).
Results: 64 (84,2%) eyes had no erosion during the follow-up time. 12 (15,8%) eyes had a relapse of corneal erosion. Six of them had a second treatment with the excimer laser, after which 4 eyes had no further erosion. So the total relapse rate decreased to 10,5%. There was no difference between eyes with surgical pretreatment and eyes only treated with eyedrops and ointment.
Conclusion: The relapse rate of 10,5 % shows that PTK could become the prior method to treat pesrsistent recurrent corneal erosions, especially since 10 of the patients with recurrence remarked an improvement of their subjective complaints.
