Establishing an International Uveitis-Expert-Networking Group

Objective: Diagnosis and therapy of chronical diseases, as e.g. uveitis, often need a lot of experience. The goal of this project was to establish an international data exchange network in the field of uveitis, with the possibility to discuss problem cases in an expert forum.
Methods: That for we installed a platform, where these cases could be presented and discussed. Furthermore we had to create software for easy and effective data transfer (data, images, video sequences) and to optimize hardware for compression without loss of quality.
Results: We established a web based platform that allows to discuss and introduce cases. The platform helps on acquiring and installing components to the members personal system allowing communication or image acquisitation via the internet or via ISDN. This includes software for acquiring, viewing, handling and transferring video sequences and hardware for compressing and acquiring video sequences. The software is developed in our laboratory for telemedicine applications. A video viewer and editor which is linked to an internet mail transferring module enables easily to send the video directly to a partner or the whole group. The hardware enables to connect the system to a common analogue video device (video player, slit lamp, SLO, fundus imaging system, etc). It converts the signal to a digital video sequence on the PC using an effective compression mode (MPEG technology) for sending the sequence. Hardware requirements are a PC Pentium 133 Mhz or more, board with Intel based chipset, 32 MB RAM, 4 MB graphics, ISDN adapter/internet connection. At the time the community consists of app. 20 members.
Conclusion: To our knowledge the International Uveitis-Expert-Networking Group is the first international data exchange network for diseases of the eye. Networks for other ophthalmological sectors are planned. Financial problems have to be solved before opening these kind of telemedicine to a larger group of patients.
