Semifluorinated Alcanes as Long Term Vitreous Tamponade

Introduction: Vitreous substitutes with a specific gravity greater than water promise advantages in the treatment of inferior retinal pathologies. The results of experimental and clinical studies using different semifluorinated alcanes are presented.
Material and Methods: A) O44, O62 and OL62HV were injected in rabbit eyes after mechanical vitrectomy and creation of an inferior retinal hole. The eyes were observed up to 3 months. B) In a clinical pilot study O62 was evaluated as an intraoperative tool and O44 and O62 were used as a longterm tamponade in patients with PVRdetachment and retinal tear or retinectomy in the inferior quadrants.
Results: A) The animal study revealed good intraocular tolerance, a slight epiretinal foam cell formation and Muller cell hypertrophy. In contrast to OL62HV, dispersion of O44 and O62 was observed, beginning in the first week and reduced fundus visibility. B) O62 facilitated macular rotation and allowed a direct exchange to silicone oil. O44 and O62 offered an adequate tamponade and reattachment of the inferior retina.
Conclusion: The class of semifluorinated alcanes are promising additional tools in vitreoretinal surgery. For routine clinical use an exact knowledge of the different chemical properties of each alcane is required.
