Limbal relaxing Incisions (LRI) - correction of a high astigmatism in cataract surgery

1996 J. P. Gills presented the LRI as a possibility for the correction of astigmatisms between 0,5 an 3,0 dioptres. We used a simplified Gills- Nomogram and performed the LRI in cataract surgery. For the LRI the conea was incised with a diamond knife at the limbus - parallel with it - 600 µm deep.
Patients and method: So far 27 patients with a preoperative astigmatism between 1,5 and 5,5 dioptres (mean 2,7 ± 0,96 dioptres) were included in this study. The average age was 77,5 ± 9,4 years. The Limbal Relaxing Incisions were done in the steepest meridian. The tunnel incision for cataract extraction (Phakoemulsification) was performed via one of the incisions.
Results: The absolut astigmatism could be reduced from an average of 2,4 ± 0,7 dioptres to 0,7 ± 0,5 dioptres. The reduction of the astigmatism - calculated according to Jaffe and Naylor - was 1,9 ± 0,9 dioptres after 6 weeks.
Conclusion: Limbal Relaxing Incisions are a possibility for the successful reduction of a high preoperative astigmatism in cataract surgery.
