Atopic keratoconjunctivitis: Cause for conjunctival carcinoma?

1Kallen C. J., 1Reinhard T., 1Cartsburg O., 2Böcking A., 1Sundmacher R.

1Eye Hospital and 2Institute of Cytopathology, Heinrich-Heine University, Düsseldorf

Objective: In Germany, the incidence of conjunctival carcinoma approximates 1.2/1 million/year. In 2000, 5 patients with atopic keratoconjunctivitis developed conjunctival carcinoma in the Heinrich-Heine University. Since October 2000 all patients with atopic keratoconjunctivitis, therefore, have been monitored via cytology and cytometry.
Patients and methods: Sixteen patients have been examined so far. The age of 9 male and 7 female patients averaged 45.8+/-12.7 years. The patients had suffered from atopic disease for 31.6+/-16.7 years. The eyes had been involved for 8.2+/-8.5 years.
Results: Conjunctival carcinoma was diagnosed in 2/16 patients. In one patient the disease was invasive, in the other it was graded as carcinoma in situ.
Conclusion: The detection of 2 conjunctival carcinomas in 16 patients supports the suspicion of an elevated incidence of the disease in atopic keratoconjunctivitis. A larger group of patients has to be followed in order to concretize this suspicion.
