Background: Spindle cell carcinoma of the conjunctiva is a rare malignant tumor. Local recidiv and tendency to intraocular growth are known.
Case report: A 75-year-old woman presented with a small painful episcleral tumor following cataract surgery, vitrectomy and silicone oil-filled eye. It was suspected of having nodular anterior scleritis, but treatment was without success. After biopsy and diagnosis of a spindle cell carcinoma followed the exenteratio of the orbital tissue. Histological findings were spreading of conjunctiva, episclera, ciliary body, choroid, retina and vitreous. There were no metastasis found three months after this surgery.
Conclusion: Thinking of aggressive malignant tumors of the conjunctiva is too necessary in postoperative scarred conjunctiva with new painful and increasing alterations. Doubtful findings should be under histological examination early.
