Collagen-Implant-Canalostomy, observation of 98 eyes

Background: The deep sclerectomy enjoys currently great popularity.
We report about our experiences.
Patients and methods: In 98 eyes of 89 patients with chronic open
angle glaucoma (OWG) in the age of 74 years (SD 10,5) a deep sclerectomy
with collagen implant was performed. The pre- and postoperative results
of the intraoculare pressure (IOP) were compared. As sufficient treated
eyes were valid with an IOP < 21 mm Hg.
Results: At the follow-up on average of 8.1 months (SD 3.1) the
preoperative IOP could be lowered from 23.6 mm Hg (SD 8.3) on 16.6 mm
Hg (SD 3.96). A postoperative medication was required at 52 of the 79
eyes at follow-up. 21 eyes postoperatively showed a conjunctival bleb,
which persisted only in 3 eyes. One patient showed a transient flatt anterior
chamber without choroidal detachment. It came to anterior synechiae in
2 eyes. Main risk factor for a postoperative glaucoma medication was an
IOD > 17 mm Hg within the first 3 days after the operation.
Discussion: The Collagen-Implant-Canalostomy is an effective and
save procedure in the treatment of the chronic open angle glaucoma. In
95% of the cases the IOD was normal. It may be usefull to place the collagen
device on the ciliary body to avoid anterior synechiae.
