Incidence of retinal detachment after clear-cornea phacoemulsification

Objective: There are different reports about the incidence of retinal detachment after cataract surgery. They differ between 0.3% and 3.6%. Because exact numbers are important for obtaining patients consent for the operation and for medical certificates, we had a look on our patients from the year 1996.
Methods: In 1996, 1449 phacoemulsifications with intraocular lens implantation were performed in the university eye hospital Giessen. We documented all ocular diseases, axial length, intraoperative complications and status of the retina before and after operation. Concerning informations of the long-time status of the patients (mean age 78 years, mean axial length 23.34mm, 243 eyes; lOL types: 175 silicone, 45 PMMA, 23 hydrogel), we contacted their eye doctors. The mean follow-up was 3.2 years.
Results: One of the eyes we operated 1996 had undergone surgery because of retinal detachment before the catarct operation. 4 patients had undergone surgery because of retinal detachment on the fellow eye. 12 eyes showed peripheral retinal degeneration, 3 of them were treated with laser coagulation before they came to the catarct operation and 7 after the operation. In 3 cases, an anterior vitrectomy because of rupture of the posterior capsula with prolaps of vitreous body was necessary. On 63 eyes, a YAG-laser capsulotomy was performed postoperatively. So far, none of the eyes we have informations about developed a retinal detachment after the catarct operation.
Conclusion: Up to now, we did not find a higher risk for retinal detachment after phacoemulsification in the group of patients we operated 1996 compared with the reports in literature.
