Curved paired T-cut for the correction of high idiopathic astigmatism

Background: Beneath glasses or contact lenses for the correction of an idiopathic astigmatism surgical techniques can be applied. Is the astigmatism higher than 4 D lasersurgery (PARK or Lasik) should not be used. For higher amounts of astigmatism curved paired T-cuts can be used.
Patients and method: Curved paired T-cuts were performed at 18 patients (31 eyes) between 1997 and 2001, at 17 patients with a high hyperopic astigmatism, at 1 patient with a myopic astigmatism. The surgery was done in topical anaesthesia, the optical center was 6.5 mm, the cut of 90% depth was between 3.5 and 5.5 mm.
Results: The spherical refraction could be reduced from 5,2 D (2,5 - 7,5) to 2,5 D (0,5 - 4,25); the cylinder from 6,1 D (4,5 - 8,5) to 2,5 D (1 - 4,25). The s.c. visual acuity improved from 0,06 (0,05 - 0,15) to 0,30 (0,05 - 0,8); the c.c. visual acuity from 0,6 (0,1 - 1,0) to 0,65 (0,3 - 1,0). The postoperative refraction in all cases was stable after 12 weeks (normally after 4 weeks). One patient was retreated twice because of undercorrection. 5 patients complained about blurred vision and 11 about foreign body sensation.
Conclusion: Curved paired T-cuts are a good and safe technique for the reduction of a high idiopathic astigmatism. Further refractive procedures like PRK or Lasik can be performed after the keratotomies.
