Laser thermal keratoplasty (LTK) for the correction of low to moderate hyperopia

To correct hyperopic refractive errors corneal coagulation (laser thermal keratoplasty) may be used to heat and shrunk peripheral corneal tissue which produces a central corneal steepening. Histological and ultrastructural examinations have shown superfical epitheal lesions, injury of keratocytes and stimulation of a circumscripted wound healing response after LTK. Mainly type I collagen (procollagen) is produced. In 100 eyes of 92 patients a noncontact Holmium:YAG LTK (Sun 1000, Sunrise Technologies, CA, USA) was performed and patients were followed for 2 years. The application of two laser rings (each with 8 octagonal located laser spots, diameter 600 µm) in a radial fashion on the 6- and 7 mmcorneal zone achieved in patients older than 40 years up to +3.0 dpt of correction. Disadvantages of the procedure are initial overcorrection and regression over time. LTK is an easy, safe and quick treatment modality for presbyopic hyperopia (up to +2.5 dpt) for the patients as well as for the physician.
