Anatomical Reattachment of the Retina after Retinopathy of Prematurity Stage 5+

Objective: Reattachment of the retina after total retinal detachment (stage 5+) is unusual in retinopathy of prematurity (ROP). We present a preterm infant with reattachment of the retina in both eyes after complete funnel shaped retinal detachment although laser coagulation of the avascular retina in stage 3+ ROP and retinal detachment surgery using an encircling band in stage 4+ ROP had been performed.
Case report: In a female preterm infant (gestational age 26 weeks, birth weight 725 g) laser coagulation of the avascular retina was performed when ROP reached threshold disease. As ROP progressed to partial retinal detachment (stage 4B+) this baby was transferred to our hospital. Though we attached an encircling band in both eyes ROP progressed to stage 5+ with an open funnel. No further progression of the funnel shaped detachment was observed when vascular proliferation receided in month six of life. At the same time massive subretinal exudates occurred. After another four months and absorption of the exudates and the subretinal fluid the retina was reattached in both eyes (with some dragging in the left eye).
Conclusion: Complete anatomical reattachment of the retina in ROP may occur even after funnel shaped total retinal detachment (stage 5+ ROP).
