Herrick Lacrimal Plugs with Consequences

Case report: Report on the application of Herrick Lacrimal Plugs in a 55- year-old patient who had been suffering from dry eye syndrome. After a 10- year period with unsuccessful trials of tear substitution she desided for the bilateral implantation of Herrick Lacrimal Plugs in US. Initially symptoms improved for a limited period of one year. Then epiphora and recurrent inflammation of the lacrimal system occured on both sides. Probing of the descending lacrimal system revealed a bilateral occlusion at the level of the C. communis. During surgery granulation tissue surrounding the lacrimal plug was detected on the left side. On the right scar tissue with total occlusion of the C. communis and degradation of the plug material with incorporation into the tissue of the lacrimal system was found. Demonstration of the surgical procedure and histological findings will be provided.
Conclusion: The use of lacrimal plugs implanted in the punctum is a well established method for the treatment of patients with dry eye syndrome. Herrick Lacrimal Plugs as used in this case can not recommended.
