Controlled draining in treatment of serous retinal detachment caused by optic nerve pit

Since 1994 we use laser retinopuncture for treatment of cystic maculopathy, serous retinal detachment and macular hemorrhages. Aim of the study is to develop controlled draining on the basis of laser retinopuncture employing natural intravitreal circulation in combination with retinal lasercoagulation.
Patients and methods: 5 eyes of 5 patients aged from 18 to 40 years with serous macular detachment caused by optic nerve pit were treated. Mean visual acuity was 0.05. Three eyes underwent controlled draining of subretinal cavity: after puncture of the inner wall of subretinal cavity with YAG-laser (1064 nm; E=7.1 mJ, n = 1-2) tunnel lasercoagulation of retina was performed. Its aim was to direct the flow of subretinal fluid into the vitreous, bypassing the macula.
Results: In 3 patients after controlled draining the macula was attached and visual acuity improved up to 0.15-0.2. In 2 cases after barreer lasercoagulation alone retina remained detached and visual functions did not improve.
Conclusions: Control of draining process after laser retinopuncture for serous macular detachment is possible. Further investigations in this area are necessary.
