Nonpenetrating deep Sclerectomy: Short-term Results

Purpose: The aim of this study was to evaluate the effect of deep sclerectomy (DS) in patients with primary open angle glaucoma.
Setting: University hospital K.B. "Sestre milosrdnice"
Patients and Methods: We prospectively studied 28 eyes in our uncontrolled primary open-angle glaucoma patients that underwent deep sclerectomy trabecular filtration surgery. 18 eyes underwent deep sclerectomy alone and 10 eyes combined DS and phacoemulsification (PHACO) with posterior chamber intraocular lens (IOL) implantation.The technique of DS is to deroof Schlemms chanel and create trabeculo-Descemetic membrane. The mean follow-up was 8,7 months (6 to12).
Results: The mean preoperative intraocular pressure (IOP) was 23,9 ± 3,2 mm Hg (18 to 28) and average number of 2,2 topical antiglaucomatous medications. At the last follow-up in 8,7 months (6 -12) mean postoperative IOP was 16,8 ± 4,4 mm Hg (10-24) and average number of topical medications was 0,5. Visual acuity was unchanged in DS cases and improved in 98% phaco-DS cases. The main intraoperative complication was a microperforation of trabeculo-Descemetic membrane and later postoperative complications were rare.
Conclusions: DS and phaco-DS are effective and safe surgical procedures of treatment of simplex glaucoma or coexisting cataract and uncontrolled glaucoma.
