Choroidal Neovascularization: A seldom complication in focal laser photocoagulation in exsudative diabetic retinopathy

Objective: Fibrotic scars and choroidal neovascularization (CNV) are known as rather seldom complications of fokal laser photocoagulating the exsudative diabetic retinopathy.
Methods: In two cases of patients with exsudative diabetic retinopathy we describe the etiology of occurring CNV after focal laser photocoagulation. We also discuss methods of prevention and therapies of this complication.
Results: The CNV occurred in the first case ten years, in the second case 6 months after focal parafoveal laser photocoagulation was done in clinical significant macular edema according to the ETDRS citeria. In one case the CNV was located parafoveal. Focal laser photocoagulation of this area could not prevent subfoveal spreading. Due to V. A. less than 20/200, to low for photo dynamic therapy (PDT), the CNV remained untreated in the second case.
Conclusion: Subretinal neovascularization is usually known to appear when focal laser photocoagulation is overdosed. Instead of this these cases show that these neovascularizations can suddenly occur after some years of uncomplicated history, even if there is no sign of age-related macular degeneration (ARMD) in the other eye.
