Reproducibility measurements of the retrobulban vessel system with color Doppler imaging

Objective: It is surmised, that a changed glaucomatic function loss, besides other factors for e.g. a higher intraocular pressure, is also caused by a changed haemodynamic of the retrobulban vessel system. This could be proved by various studies in which patients with glaucoma had a changed retrobulban blood velocity. During the study reproductive measurements should be performed for the establishment of the color Doppler imaging, which is used to measure the retrobulban vessel perfusion.
Patients and methods: One eye each of 22 patients from 6 different age groups (18-74 years, Median 47 years) were examined. During this study the Visus, the intraocular tension, the pulse and the blood pressure were although examined. Using a color Doppler imaging, type Siemens Sololine Elegra®with a combined soundhead (7.5L40), the arteria opthalmica, the arteriae ciliares breves and the arteriae ciliares longes of each patient were measured 6 times. Each measurement could be documented digitally by this apparatus. The reproducibility measurements were evaluated by the calculation of the intraclasscoefficient. The age correlation was discovered by the calculation of the Spearman-Rank-correlation coefficient. For the reproducibility measurements the pulse amplitude, end diastolic velocity, maximum systolic velocity and the resistance of the vessels (RI-Resistance Index) of the arteriae ciliares and the arteria opthalmica were evaluated.
Results: The intraindividual measurements reflects a good reproducibility of the above mentioned indicators. The intraclasscoefficient of the arteria opthalmica were 0.92, the arteriae ciliares longes 0.88 and the arteriae ciliares breves 0.82. The age correlated increase of the vessel resistance could not be proved by the measured vessels (p>0.05).
Conclusions: The color Doppler imaging is a suitable process for the reflection of the retrobulban blood flow condition. The significant reproductive results are the presupposition for the use of the color Doppler imaging in the glaucoma diagnostics as well as in therapy studies.
