Reversible congenital ectropion in congenital ichthyosis

Background: In rare cases of congenital ichthyosis a congenital ectropion can result from the formation of a collodion-typ skin.
Methods: We describe the pathomechanism, differential diagnoses, therapy and history of a case with congenital ectropion.
Results: A male newborn presented with bilateral upper and lower lid ectropion resulting in lagophthalmos in primary position. The child was delivered in the 34th week of pregnancy by cesarean section. The entire skin of the newborn had a parchment like appearance and was keratinised. On lid closure the ectropion worsened due to a functional shortening of the anterior lid lamella. Further findings, such as fish mouth shaped lips, an increased tendency of the skin to develop fissures with desquamation as well as the absence of any other internal malformations helped to establish the diagnosis of a congenital ichthyosis (synonymous: collodion baby). Medical treatment consisted of dexpanthenol ointment, application of antiseptic solution and antibiotic eyedrops. Within a few days the collodion skin became detached and the ectropion regressed spontaneously. Complete lid closure was achieved on the 9th day of life. No corneal complications occurred.
Conclusion: Temporary ectropion in congenital ichthyosis can result as a functional shortening of the anterior lid lamella. With supportive skin care this may resolve without any further need for surgery.
