Giant tear retinal detachment

Giant retinal tear is defined as a retinal break which extends at least 90 degrees around the circumference of the fundus. Because of large area of bare pigment epithelium giant retinal tears have high tendency to develop proliferative vitreoretinopathy (PVR) and worse prognosis compared to rhegmatogenous retinal detachment. In this video a surgical procedure to treat giant tear retinal detachment is presented. "Simple" cases of retinal detachment are presented, as well as cases complicated with severe PVR. Both pars plana vitrectomy combined with gas tamponade and silicone oil surgery are illustrated. In our clinical experience uncomplicated giant tear retinal detachment as well cases complicated with PVR can be successfully repaired. Late anatomic and functional results are favorable, even in eyes with severe PVR.
