Comparing bilateral multifocal , bilaterl bifocal and one eye bifocal and the other monofocal IOL implantation

Purpose: To compare visual abilities in patient with bilateral monofocal, bilateral multifocal and one eye monofocal and the other one multiofocal IOL implantation
Method: The fist group, patients with a monofocal intraocular lens in both eyes consists of 14 patients, the second group with multifocal lens in both eyes of 16 patients and the number of patients in the third group with a combination of a multifocal and a monofocal IOL is 17. The multifocal lens was a diffractive bifocal 811E (Pharmacia), as monofocal lens the 811 C (Pharmacia) was used. Visual acuity without correction and with best correction was tested monoculary and bilateral. Halo size was determined using a computer program Glare&Halo (Tomey) for each eye and bilateral. Further the glass wearing behaviour had been recorded.
Results: Visual acuity without correction was in the first group monoculary Snellen 0.57 ± 0.19 and with correction 0.98 ± 0.16 and 0.60 ± 0.17 and 1.02 ± 0.14 binocular. The halo size testing monoculary was 0.24 square degrees ± 0.10 and 0.25 ± 0.11 binocular. In the second group the visual acuity was monoculary 0.55 ± 0.14 and 0.95 ± 0.13, binocular 0.58 ± 0.12 and 0.98 ± 0.12. Halo size was 1.04 ± 0.5 unilateral and 1.01 ± 0.51 bilateral. In the third group the eyes with the a monofocal lens achieved 0.54 ± 0.16 and 0.97 ± 0.14, with the multifocal IOL attained 0.60 ± 0.15 and 0.97 ± 0.14, bilateral 0.62 ± 0.10 and 1.0 ± 0.11. The halo size was for the monofocal eyes in this group 0.23 ± 10, for the multifocal eyes 1.02 ± 0.42 and 0.98 ± 0.38 bilateral.
Conclusion: The combination of a monofocal IOL implantation in one eye and a bifocal IOL implantation in the other eye is a satisfying possibility for cataract patients allowing a trouble-free short time reading in near distance.
