Analysis of congenital dacryostenosis in consideration of mucoceles

Obstruction of the distal aspect of the nasolacrimal duct is common occuring in 30 % of newborns. Dacryocystitis or mucoceles are rarely seen in newborns, because of spontaneous rupture of the membrane soon after birth due to mechanical stress. This will happen in 90 % of all cases. Persisting of the membrane will lead to an acute dacryocystitis which can become chronical. Congenital mucocele refers to a distended lacrimal sac present around birth.
Patients and methods: A retrospectiv analysis congenital dacryostenosis who where treated over a 4 years period was performed. 256 cases(130 girls, 126 boys) were included. Age ranged from few days to 31 month.
Results: Bilateral dacryostenosis was found in 20 % of cases. Congenital mucocele occurred in 11 cases. There were a strong female preponderance(10 cases). Unilateral mucocele was found in 10 cases. Syringing and probing was required. Repeated intervention was necessary in 10 cases. One mucocele required probing with silicone tube placement.
Conclusions: A congenital mucocele is one differential diagnosis possibilitiy in cases of blue swelling in the area of medial canthus at newborns. An immediatly therapy is necessary. Syringing and probing of nasolacrimal ducts should be realized in the first 6 months after birth, because the treatment can be performed in local anaesthesia.
