Chronic-recurrent amaurosis fugax with up to 100 events per day with vague reason

Case: A 53 years old female patient is suffering from chronic-recurrent events of a amaurosis fugax in the oculus ultimus with up to 100 events per day. The number of the events is increasing from the beginning in the age of 10. Furthermore she has sometimes episodes of headaches. A neurological examination did not detect any reason. A Horton´s disease was excluded. The other eye was blinded by a chronic iritis, and a funduscopy is impossible. The examination of the stricken eye shows no suspect findings except some dilated veins. During a 10 second attack the venous vessels show a huge dilatation and winding and the optic disc margins is blurred (Video). The blood flow in the A. centralis retinae is moderate decreased in case of amaurosis fugax. Other intraorbital vascular abnormities were not found. Both CT and MRI angiography in highest resolution has shown any intracranial or intraorbital pathology.
Conclusions: A reason of chronic-recurrent attacks of temporary blindness has not yet been found. Possible explanations could be an unknown vessel anomaly or an atavistic muscle. An accordingly diagnostic detection failed in doing.
