Intracorneal Ring Segments (ICRS) in Keratoconus

Background: Perforating keratoplasty is the ultima ratio in advanced keratoconus with impossibility of correction by spectacles and unablity of contact lens wearing. ICRS can occasionelly help to restore the corneal ability of fitting contact lenses or the possibility of correction by spectacles.
Patients: In December 2000, we implanted ICRS in two eyes of two patients (1 female, 17 years; 1 male, 58 years) with advanced keratoconus. Both patients showed an intolerance of wearing contact lenses. A keratoplasty lay ahead. In both patients, ICRS was implanted in retrobulbar anesthesie.
Results: Postoperatively, best contact lens corrected visual acuity increased from (Fingerzählen) to 0.25 and from 0.05 to 0.4. In the male patient, the corneal curvature became significantly flatter. The young patient showed even preoperatively a central cloudy cornea with a speedy progredient keratoconus; postoperatively, the central cornea became steeper, but a contact lens fitting was possible.
Conclusion: In our both patients with keratoconus, corneal surface could be changed in that way, that a contact lens can be fitted or a correction by spectacles can be performed. Keratoplasty could be delayed. These cases confirm the cases described in literature. The durance of the effect is not yet known.
