Cataract operation with small pupil and posterior synechia without manipulation on the iris

Objective: The film shows a cataract procedure with small pupil without manipulation on the iris. In contrary to well known approaches where the pupil margins are forced by retractores this procedure tries to live without manipulation of the iris.
Method: A rhexis is performed of the pupil size and after hydrodelimination the phako as well as the A/I is done through this small entry. Under a little force the 5,5 mm diameter IOL is introduced in the capsular bag. Wound closure is necessary when using this approach.
Results: An examination shows that the IOL sits in the bag. The visual acuity raised from 0.2 to 0.7 as there were preexisting problems caused by the iritis at the vitreous and the retina. The essential steps of the whole procedure are shown in in the film.
Conclusion: The shown procedure is an alternative to stretching the iris for proper view into the region of interest during the cataract operation. The disadvantage of this method is the reduced visual overview over the whole area of operation. The advantage is the minimum of trauma that is done to the tissue.
