Effect of isovolemic Hemodilution on retrobulbar Hemodynamics in disorders of ocular Perfusion

Objective: The therapeutic benefit of isovolemic Hemodilution treatment (IHT) in ocular vessel occlusions is subject of debates. This study aims at the evaluation of IHT with respect to retrobulbar Hemodynamics.
Methods: In a prospective study we examined 54 eyes from 27 patients with a disorder in ocular Perfusion (OPS) (17 male/10 female, average age 62.8 years, 11 arterial OPS, 18 venous OPS) utilizing Doppler ultrasonography (Toshiba SSH 140A) prior to and 10 days after initiating the IHT. The blood flow measurement of the A. and V. centralis retinae, A. ophthalmica, Aa. ciliares, and V. supraorbitalis has been carried out both on the affected and the not affected eye in m/s. The target hematocrit value for the IHT was HK </= 0,37. The statistical analysis took place by applying the Wilcoxon's test for blindfolded samples.
Results: After applying IHT a significant increase of blood flow velocity in the A. ophth. and the Aa. ciliares (p<0.05) could be observed in arterial OPS. In venous OPS a significant increase (p<0.05) of blood flow was recognized in all examined vessels, except for the A. ophth. The Resistance Index (RI) in arterial OPS remained unchanged after IHT.
Conclusion: After the IHT both in arterial and venous OPS a significant improvement of ocular Hemodynamics could be obtained.
