Choroidal haemorrhage and posterior Sclerotomy after complicated Cataractextraction: a case report

Patient: A 83 year old diabetic women lost her independence to lock after herself due to reduction of visual acuity in her only seeing eye. That eye had an acuity of 0.3, Myopia, dens Nuclear Cataract with Pseudoexoliativ Glaucoma and Phacodonesis. The other eye had, with counting fingers and projection of light, no useful function.
Surgery and follow up: After a normal Phaco-Chop procedure a spontaneous posterior capsular tear with vitreous loss occurred due to choroidal haemorrhage. We carried out an anterior vitrectomy followed by implantation of an anterior chamber lens. The follow up showed an extensive haemorrhagic choroidal detachment with visual acuity of counting finger an incorrect light perception. To release the haemorrhagic chorioidal detachment we carried out a posterior sclerotomy under general anaesthesia. As a result the chorioidal detachment settled very well.
Results: The follow up has been without complication. The uncorrected visual acuity (UCVA) increased to 0.7 and the Patient gained her independent life again.
Conclusion: It is worth providing a choroidal haemorrage because in some cases useful function can be kept.
