A case of successful treatment of the bacterial ulcer after surgical removal of the pterygium type III with phototherapeutic keratectomy

Case report: A 60-year-old white woman presented to the Hospital Microsurgery of Eye on May 24, 2000 with a complaint of pain, burning, photophobia and redness in the right eye. On examination her VA without correction was 0.3 in the right eye and 0.09 in the left eye. With correction VA improved to 0.5 with +1.0 +3.0 x 640 in the right eye and to plano + 6.0 x 1000 in the left eye. Slit-lamp microscopy was remarkable for corneal ulcer at the nasal periphery with infiltrated adges. Two months ago she was underwent surgical excisional procedure of the pterygium type III on her right eye After 1-week failure of therapeutic treatment with antibiotics she was underwent phototherapeutic keratectomy applied on the surface of the ulcer in the right eye with the Nidek EC-5000 excimer laser using masking fluid to smooth the infiltrated edges of the ulcer. Ablation diameter was 5.5- mm with 6.5-mm transition zone size and an estimated ablation depth was to remove all necrotic tissures to slightly opacificated cornea.The healing process completed in a three days resulting in resolving of the pain, photophobia and irritation of the eye. At the long-term follow-up (6 months after PTK) she reported the subjective improvement of her VA though her VA remained the same. On slit-lamp examination the nasal corneal appearance was dramatically improved with almost clear peripheral cornea and vascularization resolved.
Conclusion: The surface treated with excimer laser is very smooth and even transparent depending on the depth of the keratectomy. This case report has showed that excimer laser treatment for therapeutic reasons in bacterial ulcer is effective method coping with the rare complication in pterygium surgery.
