Tumor of the lacrimal gland: a late complication after levator aponeurosis repair as a treatment for ptosis due to Blepharochalasis Fuchs?

Objective: A 48-year-old patient had already suffered a shortening of the levator aponeurosis for bilateral ptosis and Blepharochalasis Fuchs 11 years ago in a foreign hospital. He complained about newly increasing bilateral ptosis and a progressive tumor of the right superior-lateral palpebral region.
Material and methods: Clinical examination revealed lid apertures of 10, 6 and 0 mm (R.E.) and 11, 6, 1 mm (L.E.). 30°-visual field disclosed defects in the upper peripheral regions. Myasthenia gravis was excluded by testing with edrophonium and curare. Elevating the lateral upper and lower lid of his right eye a conically shaped, rose-coloured tumor with tuberous surface protruded. Computer tomographic and nuclear magnetic image scans presented increased lacrimal glands with a cystoid formation on the right side. No infiltration of muscle, fat or bony parenchyma was seen.
Results: A Fasanella-Servat procedure was performed and the tumorous mass removed. Histologic examination revealed a simple cyst of a lacrimal ductule.
Conclusions: The palpebral tumor can be explained by an obstruction of the lobule's orifice of the lacrimal gland and was probably initiated by the primarily performed ptosis-operation.
