| Phako - review and new developments Wilhelm F. Martin-Luther-Universität, Klinik und Poliklinik für Augenheilkunde, Halle/Saale Summary: After the first report about phakoemulsifikation by Kelman
the technique was developed rapidly. The kapsulorhexis caused always new
creations in endocapsular lens surgery. Main topic of the development
were also the care for the cornea endothelium, small incisions and short
duration of the surgical procedure. After the divide-and-conquer-technique
came the chop technique modified by bevel-down use of the tip and the
cho-chop-flip method with the advantage of more effective application
of energy on the lens nucleus. The possibilities in this field are limited.
Therefore the surgeons locked for alternatives. Most developed is the
laserphako - particular used routinely in the clinics, but limited. By
separating irrigation and aspiration the lens removing only through the
paracentesis is possible. The waterjet additionally allows to reduce the
secondary cataract. Similar mechanical methods for phakolysis - "endolentikular
fragmentation" and "catarex" will get their place in the
cataract surgery of the future. The careful procedure with extremely small
incisions of anterior chamber and lens capsule show the place in the time
of lens refilling by injcectable intraocular lens. |
