Measurement of macular pigment density in patients with age-related macular degeneration

Purpose: There is a growing body of evidence to indicate that oxidative damage plays a role in the pathogenesis of age-related macular degeneration (AMD). Consequently, the possibility that the antioxidant properties of macular pigment (MP) confer protection against AMD has been postulated. A proved protective effect of MP may be of therapeutic value, as it has recently been reported that human MP can be modified with dietary supplementation. This study was conducted to measure the MP density in patients with AMD.
Methods: The measurement MP density performed with a scanning laser ophthalmoscope. First, fundus reflectance images at 488- and 514 nm argon laser wavelengths were recorded. Thereafter, these images were corrected for eye movements, the images converted to log reflectance maps and digitally subtracted. The resulting images are functional images of MP density. After correction for the difference in the MP absorption at 488 and 514 nm, for the lens density, and for differences in the intensity of the laser light at different wavelengths MP density was evaluated in the fovea.
Results: Macular pigment density was quantified in 34 healthy subjects and 33 patients with exudative AMD. MP density in the fovea was significantly reduced in patients with exudative AMD (p<0,05). Relative MP density was 0,32 ± 0,20 in healthy subjects and 0,22 ± 0,10 among patients with AMD.
Conclusion: Measurement of MP density can be performed by analysing reflectance images recorded with scanning laser ophthalmoscope. Our data show a reduction of MP density in patients with exudative AMD. Further studies are needed to test if the reduction of MP densities a primary or secondary phenomenon in exudative AMD.
