Amblyopia of the contralateral eye in unilateral congenital fibrosis

Background: Amblyopia of the ptotic and the contralateral eye in spite of head posture due to complex motility disorders are well known. This case report shows the importance of the exact ophthalmological and orthoptic examinations to avoid amblyopia following even mild ptosis.
Patient: A five year-old girl was referred to our clinic because of strabismus and ptosis of the left eye. The examination showed a mild reduction of elevation of the left eye and reduction of the visual acuity of the right eye. Visual acuity of the left eye was normal. We diagnosed mechanical dysfunction of the musculus rectus inferior of the left eye in terms of congenital fibrosis. Due to the preferred fixation with the left eye an amblyopia of the right had developed. Consequent occlusion therapy lead to an improvement of the visual acuity to 0.8.
Conclusion: Even mild motility disorders or ptosis require an exact ophthalmological and orthoptic examination to avoid amblyopia.
