Medium-term results after deep sclerectomy

Objective: To evaluate the intra-ocular pressure in eyes with glaucoma after deep sclerectomy.
Patients and Methods: Prospectively the intra-ocular pressure of 85 eyes (67 patients) with glaucoma was studied after deep sclerectomy. The mean follow up was 8,9 ±5,9 months. Mitomycin C was applied in 14 eyes with high risk af fibrous reaction and 11 eyes had a combined cataract procedure. The deep sclerectomy was performed without using material of high viscosity or a collagen implant.
Results: The mean preoperative pressure of 25,2 ±8,7 mmHg could be lowered to 16,6 ±5,9 mmHg (p<0,0001). The number of glaucoma medications was reduced from 2,2 ±1,1 to 0,4 ±0,9 substances. 25% of eyes needed glaucoma medication again. Perioperatively 11 eyes had a Seidel-phenomenon, 9 eyes a chorioidal detachment and 8 eyes a temporary hyphema.
Conclusion: The deep sclerectomy lowers the intraocular pressure as good as the standard trabeculectomy and the complication rate is lower during the early postoperative weeks. It seems that during the follow up a higher number of eyes need glaucoma medication again.
