Laser Extraction of Hard Cataract using a new «Racot» Machine-based on 1.44 mcm Nd:YAG Laser

In suggested videofilm we demonstrate surgical technique of hard cataracts removal with use of new laser machine “Racot” based on use of Nd:Yag laser with 1.44 mcm wavelength. After standard capsulorhexis and hydrodissection working tips are introduced into anterior chamber-irrigation-aspiration one via 2.5 mm incision and laser one via 0.8 mm incision. We form bowl defect at the center of the nucleus using 150-180 mJ energy in case of 3-4 grade density cataract. After that laser energy is transferred on the walls of bowl. Nucleus cracking lines appear and further lens division on 3-4 fragments occur, the latter are moved to aspiration hole and destroyed before entering aspiration channel. Special silicone purtle located on the working part of irrigation-aspiration tip is transferred up to lower parts of fragments for additional protection of posterior capsule. If nucleus cannot be cracked because of high-elastic epinucleus, we make lens wall as thin as possible in order to aspirate peripheral nucleus layers as single piece. When removing cataracts with high density grade (brown and brunescent nuclei) bowl technique with further nucleus cracking is used also. But it is necessary to fixate laser tip along the future cracking line during 10-15 sec. to form cracking line. We think that advantage of our technique is decrease of mechanical pressure on capsular bag because such surgical steps as multiple nucleus rotation, instrumental cracking of nucleus are eliminated. Silicone purtle enables to exclude entirely risk of spontaneous aspiration of posterior capsule to aspiration hole. So possibility of reliable removal of most difficult cataracts-such as hypermature and brunescent cataracts with thin posterior capsule and weak zonula ciliaris-occurs.
