How Accurate Does the Auto-refractor Power Refractor work in the Clinical Practice?

Purpose: The aim of this prospective study is to investigate how accurate works the autorefractometer Power Refractor in comparision with retinoscopy and established autorefractometers.
Patients / Method: 150 patients (300 eyes) were examined. Patients were initially (pupil was not pharmaceutically influenced) analyzed with the Power Refractor (PRneutral). After that a examination was followed under standardized Cyclopent cycloplegia with the Power Refractor (PRcyclo), the hand-held Retinomax (Nikon), the table-top mounted RK-5 (Canon) and retinoscopy (one observer).
Results: Difference from retinoscopy in spherical values: PRneutral: mean 1,65 ±1,55 dpt, max. 7,5 dpt; PRcyclo: mean 1,14 ±1,11 dpt, mean. 6,5 dpt; RK-5: mean 0,55 ±0,52 dpt, max. 2,75 dpt; Retinomax: mean 0,26 ±0,26 dpt, mean. 1,5 dpt. Difference in cylindric values: PRneutral: mean 0,63 ±0,7 dpt, max. 4,75 dpt; PRcyclo: mean 0,71 ±0,7 dpt, max. 3,25 dpt; RK-5: mean 0,39 ±0,33 dpt, max. 1,25 dpt; Retinomax: mean 0,39 ±0,44 dpt, max. 2,25 dpt. Difference in spheric equivalent: PRneutral: Mittel 1,53 ±1,5 dpt, max. 7 dpt; PRcyclo Mittel 1,24 ±1,26 dpt, max. 7 dpt; RK-5 Mittel 0,5 ±0,5 dpt, max. 3 dpt; Retinomax Mittel 0,3 ±0,29 dpt, max. 1,38 dpt.
Conclusions: The accuracy of measurement of Power Refractor is not as good as of the other tested devices. The obtained data from the Retinomax, the Canon RK-5 and retinoscopy are comparable. The Power Refractor may be a usefull device for screening small children because of the one meter observing distance, but for a precise refraction a retinoscopy under cycloplegic conditions is still necessary. A further study will show the valence for refractive screening.
