Bilateral Simultaneous Cataract Extraction

Introduction: Bilateral simultaneous cataract extraction has been an issue of controversial discussion over the last years. Also not performed routinely, certain indications may justify this procedure considering the benefits to the patient.
Patients and methods: In selected cases simultaneous bilateral cataract extraction with intraocular lens (IOL) implantation was performed in 90 eyes of 45 patients between March 2000 and Feb. 2002 by four surgeons. The surgery on each eye was performed like an independent procedure (fresh instruments, coats, gloves, phako handpieces etc.) The indications, complications and visual results were analysed.
Results: The most common reasons for simultaneous surgery were anticoagulation therapy (24.4%) and reduced health status (17.8%). The overall incidence of intra- and postoperative complications was 1.1%, namely one posterior capsule rupture. No major complications like postoperative endophthalmitis occurred. No bilateral complications were seen.
Discussion: In this series, the rate of complications of bilateral simultaneous cataract extraction did not exceed that of the single eye procedure. Although the overall results were positive, this procedure is not a routine surgery and has to be indicated carefully and performed as an orchestration of two consecutive but independent surgeries.
