Cataract Surgery in Patients with Glaucoma Fere Absolutum

Background: The purpose of this study was to assess the benefit of cataract surgery in patients with progressive cataract and glaucoma.
Methods: 12 patients (12 eyes) were studied prospectively. Inclusion criteria were the diagnosis of a progressive cataract and a glaucoma fere absolutum. The criteria for the diagnosis of a glaucoma fere absolutum were a 0,9-1,0 glaucomatous excavation of the optic disc and marked visual field defects in the Humphrey Field Analyzer 30-2 or 24-2 perimeter program (HFA-II, Zeiss). Preoperatively and after 5 month (2-7 month), the visual acuity (V), the intraocular pressure (IOP), the number of antiglaucomatous drugs and the visual field data were assessed. Furthermore, the surgical procedure and possible complications were noted.
Results: In 9 cases (75%) the visual acuity increased from 0,3 to 0,5. Additionally in 8 cases (66,7%) there was an intraocular pressure reduction of 3,5 mmHg (-15,2%). The number of antiglaucomatous drugs decreased from 1,3 before surgery to 0,7 after surgery.
Conclusion: Patients with progressive cataract and glaucoma fere absolutum can benefit from cataract surgery. Although the presence of marked visual field defects, an increase of visual acuity as well as a decrease of intraocular pressure can be achieved.
