Topical Anesthesia for Combined Phacoemulsification and Transpupillary Silicone-oil Removal

Purpose: To investigate the efficacy of topical anesthesia for combined cataract extraction and transpupillary silicone oil removal.
Method: In this retrospective study we included 11 eyes of 11 patients (age: 65±9, 6 females und 5 males), who presented at the University Eye Hospital in Mannheim for silicone oil removal and in whom cataract extraction was indicated. All patients were operated on by three experienced surgeons using phacoemulsification with a clear cornea incision, planned posterior capsulorhexis through which silicone oil removal was performed, and endocapsular implantation of a posterior chamber intraocular lens. All operations were performed in topical anesthesia with Oxybuprocaine (Novesine®).
Results: All 11 operations could be performed in topical anesthesia. The implantation of the intraocular lens was in the capsular bag in 10 patients, int the ciliary sulcus in one patient. The reason for the primary silicone oil instillation was a retinal detachment in 8 patients, a proliferative diabetic vitreoretinopathy in 2 patients and a persistent chronic macular hole in one patient.
Discussion: Topical anesthesia is a reliable and safe method for phacoemulsification and transpupillary silcone oil removal. The main advantage of this procedure is to minimize the anesthesiological and surgical trauma for the patient.
